This program is intended for new patients who are experiencing a financial barrier to accessing pelvic floor physical therapy at the standard rate.
Patients must complete a Community Care application.
Two new patients will be selected each month (current patients are not eligible to apply)
Applications are open all month long and close on the 15th of each month. Applications received after the 15th will be considered for the following month. Once applications are reviewed, selected recipients will be notified within 2-3 days.
If you are selected, you have 30 days from the time you are notified to book your initial evaluation. This is to ensure the program can help individuals who are currently in need of pelvic floor physical therapy.
If you are not chosen the month you apply, you can reapply the following month.
The Community Care rate is $125 for the initial evaluation and $105 for follow-up visits. This program is only valid for in-clinic appointments.
Applications are reviewed monthly based on financial circumstances, household considerations, clinical need, and ability to benefit from pelvic floor physical therapy.
No financial documentation is required. This program works on the honor system.
The Community Care rate applies throughout the patient's initial plan of care and cannot be combined with other discounts or packages. It’s important to note that this rate does not last indefinitely.
Individuals who already completed an episode of care under the Community Care Program can reapply down the road for a new episode of care, but priority each month will be given to new patients.
Community Care patients receive the same individualized, high-quality care, treatment, and time as all Sarkisian Pelvic Health & Wellness patients.
Applications are reviewed monthly, and submitting an application does not guarantee acceptance into the program.
All Community Care applications will be treated as confidential. Information provided through the application will only be used to determine eligibility for the program.
